2010 Seminar Registration Form

Pre-registration is preferred. Payment must be included with registrations by mail. If you fax (518-434-4640) or email (www.nysgfoa.org/regform.htm) your registration form, you will be invoiced. Payments should be made in advance or brought with you to the seminar. Walk-ins are accepted, there will be an additional fee of $10.00 for all Walk-ins.

Cancellations
Any cancellations one week prior to the event will be charged an Administrative fee. Cancellations received after this date will not be refunded. Cancellation notices may be mailed to NYS GFOA at the address listed below, emailed to info@nysgfoa.org or faxed to 518-434-4640.

No Shows
In the event you are unable to attend and have not cancelled in writing in advance of the seminar date you selected you are still obligated to pay.

If you are currently a member of NYS GFOA, you are only required to include your first name, last name and organization to register. If any of your contact information has changed since you renewed please complete the entire form. If you are NOT a current member of NYS GFOA, please enter all information requested on this application.
